Required Steps Before Your Appointment

  1. You must have a MN driver’s license or ID card to qualify for a Short Term, Long Term or Permanent Placard.
    Without a MN driver's license or ID card, you will only qualify for a temporary placard.
  2. Complete the Disability Parking Certificate Application (PDF opens in a new tab)
    • The top portion of the application must be filled out and signed by the person with a disability
    • The bottom 2/3 of the application must be filled out completely and signed by the physician
  • There can be no alterations on this application. If alterations are made, a new application is required.
  • Anyone can complete this transaction on behalf of the eligible individual with a disability as long as the eligible individual has signed the form.

Multiple Transactions: Only one transaction per appointment. If you need to complete more than one transaction during your appointment, choose Multiple Transactions instead.
